Positions Vacant – ACFI Coordinator

Yea & District Memorial Hospital is a small rural health service comprising 10 acute beds, 4 bay Urgent care centre, Residential Aged Care facility with a 10 bedded nursing home and 15 bedded Hostel (with aging in place). We also deliver community health services to our community with community engagement and involvement a key strategic focus in the current plan.

We are currently seeking an ACFI (Aged Care Funding Instrument) Coordinator who is enthusiastic about aged care service delivery and improvement for a fixed term 12-month position – 1 day per week.

The role key focus area is the coordination, collection, and maintenance of information to support ACFI claims within YDMH.

The position will also work collaboratively with a wide range of stakeholders internally and externally ensuring that all the ACFI requirements are met by the organisation.

Support and coordinate quality development improvements within aged care services.

We also deliver community health services to our community with community engagement and involvement a key strategic focus in the current plan.

We are currently seeking a Quality Safety & Risk Manager who is an enthusiastic about service quality improvement to coordinate this varied work across the organisation.

Minimum of two years’ experience working in a quality management role essential.

The Quality Safety and Risk Manager (QSRM) is responsible for providing leadership, direction, co-ordination and management of quality, safety and risk systems and improvements activities with Yea & District Memorial Hospital (YDMH) services.


The QSRM works collaboratively with the leadership team, clinical and non-clinical staff, and consumers to ensure that YDMH has an effective, coordinated, organisation-wide approach to the provision of quality improvement, incident and risk management, management of policies and procedures, emergency and safety management systems and accreditation process across all service areas of the organisation. 

Key role components include:

Oversee organisation wide compliance tracking systems that support effective control of compliance issues: legal compliance, department directives, risk management, incident and hazard reporting and actions, audit schedule, consumer feedback processes, and policies/procedures.

* Convening YDMH standing committees: Yea for Quality, Clinical Governance Committee, OH&S, & Visiting Medical Officer.

* Manage accreditation cycles and work with other service managers and Executive to build an organisational culture of continuous quality improvement in line with standards.
