District Nursing

Our District Nursing service provides nursing care in the homes of people living in Yea and surrounding districts. We aim to envelope persons who need our support but maybe on the edge of catchment areas and work with our neighbouring District Nursing services to ensure support is available to all.

We recognise that clients have individual needs and we attempt to identify and cater for these needs when developing a nursing treatment plan. This is done through a process of assessment and consultation.

Currently the service is available Monday to Friday 9am -4pm. With the goal of expanding this to weekend cover.

Coverage areas include Molesworth, Glenburn, Castella, Flowerdale, Strath Creek, Highlands, Kinglake & Yea areas

The District Nursing Services is operated by qualified nurses both Enrolled and Registered.

We help our patients in their own home with a wide range of conditions and situations including:

  • Wound care
  • Carer Support
  • Continence Management
  • Dementia care
  • Diabetes management
  • Medication management
  • Palliative care
  • Post-acute care
  • Wellness and Reablement
  • Carer Support
  • Referral for personal care assessment and assistance
  • Stoma care & management

 

The District Nurses can also seek help and can refer you to other services provided at Yea for example:

  • Diabetes Educator
  • Mental Health
  • Dietician
  • Cardiac support program
  • Social Worker

 

Active Service Approach

Yea & District Nursing Service supports an active service approach, helping clients remain as independent as possible.

The aim of the active service approach is to assist clients to:

  • maintain their current level of physical activity or work towards improvement
  • remain part of their local community
  • retain current skills and in some cases learn new ones
  • maintain independence

 

Please talk with the District Nurse if you have a particular goal you would like to achieve or there is a task or hobby you once enjoyed but are no longer able to manage. Together we may be able help you to achieve the things that are important to you.

How to access Yea District Nursing Service (Referrals)

There are many referral pathways and if any of the below options do not apply to you can self refer by calling or emailing, see below for details.

If you are over 65 you can contact My Aged Care on 1800 200 422 anytime from 8am to 8pm Monday to Friday and 10am to 2pm on a Saturday and discuss assessment and referral to the Yea District Nursing. They may provide you a referral code to provide to us or they may be able to refer directly to us online, after which we will make a time to come and see you.

This type of referral is necessary to comply with the Aged Care Act and ensures all older people are assessed and edible for subsided nursing support.

Without this type of referral full fee recovery billing applies. However all individual circumstances are considered.

If you are a Department of Veterans Affair (DVA) recipient, please contact them on: 1800 838 372 or provide us with your DVA number.

If you are an NDIS recipient your case worker or plan manager can refer to us, alternatively you can make contact and pass on their details to us.

If you are in hospital, the staff who are discharging you can refer you to HITH (Hospital in the home) or PAC (Post acute care) for funded short term nursing support if required.

If you have a hearing or speech impairment the Translating and Interpreting Service on 131 450 or 1800 131 450
