Yea & District Memorial Hospital’s District Nursing service provides nursing care and support to people in their own homes across Yea and surrounding communities.
Our qualified Registered and Enrolled Nurses work with each person to understand their individual needs and develop a care plan that supports their health, wellbeing and independence.
Where appropriate, we also work with neighbouring District Nursing services to help people living near the boundaries of our service area access the support they need.
Wound assessment, treatment and ongoing care.
Support with medications and treatment plans.
Nursing support to help manage diabetes at home.
Nursing support with continence management.
Support with stoma care and management.
Nursing care focused on comfort, dignity and quality of life.
Support for people living with dementia and their carers.
Short-term nursing support following a hospital stay.
Nursing care to public hospital patients in the comfort of their own home.
Our District Nursing team works with you to help you remain as independent and active as possible.

If you receive Department of Veterans' Affairs support, you can contact DVA or provide your DVA details when speaking with our team.

Your Support Coordinator or Plan Manager can refer you to District Nursing. You can also contact us directly and provide their details.
Hospital staff can arrange referrals for eligible short-term nursing support following discharge, including Hospital in the Home or Post-Acute Care.
If none of the referral pathways above apply to you, contact our District Nursing team directly and we'll help you determine the most appropriate way to access the service.
Our District Nurses can also help connect you with other YDMH services where additional support may be helpful.
The cost of District Nursing may depend on your circumstances, eligibility and the program funding your care.
People aged 65 and over may need an assessment and referral through My Aged Care to access subsidised services. Where an appropriate funded referral is not available, fees may apply.
Any consumables used are charged separately at cost price.
Your ability to pay will not restrict your access to the service. If you have any concerns about fees, please speak with our District Nursing team.
